Full-Time MDS/RAI Coordinator in Calgary
Job Description
Reporting to the Director of Care, the RAI Coordinator is responsible for supporting a long term care home through integration and sustainment of Resident Assessment Instrument-RAI process which allows for the compilation and reporting of Resident Care Outcomes and corresponding funding requirements.
Key Responsibilities
- To participate in all appropriate RAI MDS 2.0 orientation and training and subsequent planning /organizing of RAI process changes;
- To communicate regularly with RAI users, Home management team and regional management to promote RAI integration;
- To provide on-site RAI MDS 2.0 education and applicable PCC software training to all appropriate staff as a component of the Home’s orientation and ongoing staff learning plan;
- To coordinate and participate in the timely completion of Resident assessments, tracking forms, Clinical Assessment Protocols (CAPs) and Care Plans;
- To serve as a RAI MDS 2.0 resource for staff members and attend appropriate committees to support understanding of Resident outcomes and applicable care direction.
- To ensure a Resident assessment schedule is developed, completed, current and accessible to all RAI users;
- To oversee the RAI process according to home policies, provincial and regional regulations, professional colleges’ Standards of Practice and RAI requirements (see CIHI User’s Manual) and work with Home mgt team to promote best practices and compliance (LQIP).
- To provide ongoing leadership and mentoring for education, data accuracy, data submission and computer applications of the RAI process;
- To implement a Data Quality Program and coordinate monthly auditing process to ensure data accuracy, providing ongoing feedback and support
- To coordinate staff participation in AIS Systems Learning Center to promote capacity building and staff competency.
- To assist in maintaining a physical and psychosocial environment that meets the needs of the Residents;
- To deal tactfully and courteously with Residents, Residents’ families, visitors and other staff members ;
- To know and promote the Residents’ Bill of Rights ;
- To ensure Resident satisfaction through promotion of continuous quality improvement initiatives ;
- To initiate and attend in-service education programs where appropriate ;
- To participate in learning needs assessments;
- To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera’s Code of Conduct;
- To perform all other duties as assigned.
Qualifications
- Current Registered or Licensed Practical Nurse designation, with membership in good standing
- Minimum 3 years clinical, leadership or management experience in a long term care setting
- Minimum 1 year experience in the RAI 2.0 Assessor role, with active AIS registration
- Working knowledge with Outcome Scales, CAPs, RUGs, Care Planning, Quality Indicators, required
- Experience in the RAI Coordinator role an asset
- Previous experience in teaching the RAI process including coaching and mentoring staff
- Knowledge and experience with adult education strategies/ facilitation /presentations;
- Knowledge and proficiency in electronic documentation system related to clinical care
- Demonstrated interest and experience with technology and data entry;
- Demonstrated success leading new initiatives;
- Demonstration of strong oral and written communication skills;
